(Read carefully. This document needs to be completed, signed,
and notarized by a notary public)

Detailed Description of Equipment to be Financed
(DOWNLOADABLE)
(The lender will need you to put together a detailed description of
the equipment to be finance)

Personal Financial Statement
(DOWNLOADABLE)
(All owner/principals with greater than 25% ownership. Forms
should be signed and dated not more than 120 days old)

Business Debt Schedule
(DOWNLOADABLE)
(Please list all business debt, sign, and date)

3 Years Personal Tax Returns
(DOCUMENT TO GATHER)

3 Years Business Tax Returns
(DOCUMENT TO GATHER)

3 Years Business Financial Statement
(DOCUMENT TO GATHER)
